This option is perfect if you:

  • Come from a Bangladeshi, Pakistani, Punjabi, Arabic, or Hindu background and would benefit from culturally and language-specific support
  • Have a visual or hearing impairment

How do I join?

By acting now, you can be part of our programme which has helped thousands of people across England to reduce their risk of developing type 2 diabetes.

There are two ways to join:

GP referral Speak with your GP or a relevant healthcare professional to find out your eligibility and request a referral.

Self-referral If you know you’re at risk, just fill out our online form with your up-to-date blood glucose reading (HbA1c) and NHS number.

What happens next?

Once referred, your referral form will outline your tailored remote support needs, and you will be allocated a dedicated coach who can support you. Your coach will contact you to explain the programme in more detail and answer any questions you may have.

They will provide you with a schedule of online sessions available to attend.

Once you begin the programme you will start to receive:

  • Personalised advice and support from your friendly expert coach
  • Easy-to-understand information about diabetes, how it can affect you, and how to reduce your risk
  • Helpful resources on balanced eating, staying active, building lasting healthy habits, and more
  • Regular check-ins with your coach to help you stay motivated and on track

You’ll celebrate your achievements as you progress through the programme. At completion, your coach will work with you to set realistic post-programme goals to support your continued progress and help keep your diabetes risk as low as possible.
