If you have patients at risk of developing type 2 diabetes, the Healthier You: NHS Diabetes Prevention Programme provides dedicated, evidence-based support proven to reduce their risk of type 2 diabetes and related conditions, while improving overall health outcomes.
Eligible patients you refer will be invited to take part in a nine-month behaviour change programme designed to prevent or delay the onset of type 2 diabetes. Patients can choose to access the programme either through face-to-face peer group sessions in their local community or via 1:1 app-based coaching delivered by our digital partner, Liva Healthcare. The same NHS-approved curriculum is delivered across both pathways.
With the right support, around three in five cases of type 2 diabetes can be prevented or delayed through healthy weight management, improved diet and increased physical activity. The programme equips patients with the knowledge, skills and confidence to make sustainable lifestyle changes, supporting them to achieve:
- A balanced diet
- Regular physical activity
- Effective stress management
- Improved sleep
- Emotional wellbeing

Referral criteria and pathways
Our services are tailored to local needs and delivered across 13 regions in partnership with Integrated Care Systems (ICSs), Primary Care Networks (PCNs), and local authorities.
Patient eligibility criteria:
- HbA1c between 42-47 mmol/mol (6.0-6.4%) or FPG between 5.5-6.9 mmol/I within the last 12 months
- Aged 18 years and over
- Not pregnant
- No active eating disorder
How to refer
Referring a patient to our NHS Diabetes Prevention Programme is quick and straightforward. We accept referrals from GPs, practice nurses, diabetes specialists, midwives, and other healthcare professionals.
The main route for referral is directly through the General Practice clinical system, using the referral form template pre-loaded onto your clinical system.
To refer a patient:
- Confirm they meet the eligibility criteria outlined below
- Complete the referral form within your GP clinical system
- Submit the referral electronically via the system
Once a referral is received, our team will contact the patient directly to arrange an individual assessment and enroll them onto the most appropriate support pathway.
In addition to standard referrals, we also support a range of alternative referral pathways to help primary care identify and refer eligible patients:
Bulk referrals – we have tools in place to reduce administrative burden and ensure patients are contacted promptly and effectively. Please contact us directly for more information.
Open sessions – targeted outreach with practices to support referral generation. Healthcare professionals are also welcome to attend these sessions to better understand the service offer.

FAQs
How soon will patients be contacted?
LWTC will make first contact with your patient within 5 working days of receiving the referral. Patients may be contacted via text message, email, or telephone. Where possible, we will leave a voicemail that includes a direct contact number for their dedicated coach, supporting smooth and timely communication.
Please note that our team works remotely while delivering behaviour change intervention sessions, so a patient’s designated coach may not always be available to answer calls immediately.
If we are unable to successfully contact a patient after a minimum of three attempts over a 28-day period, LWTC will send a written letter to the patient’s home address requesting that they get in touch. Failure to respond may result in discharge from the programme.
To support effective communication, please ensure that all patient contact details are accurate at the point of referral, and inform patients that LWTC may contact them using multiple methods.
Can patients self register?

Yes, patients can self-register for the programme.
Please note that only individuals aged 79 and under are eligible to self-register. Any patient aged 80 or over must be clinically reviewed and referred by their GP practice.
Only patients who meet the programme eligibility criteria will be accepted following self-registration.
To self-register, patients must provide the following information to LWTC:
- Personal details (name, date of birth, age, and contact details)
- NHS number
- Most recent blood test result
- Date the blood test was taken

Healthier You: NHS Behavioural Support for Obesity Prescribing Programme
No, patients cannot self-register for this programme.
This programme forms part of the Wrap Around Care offer for individuals prescribed Tirzepatide for weight management as part of the NHS rollout. Alongside the behavioural support provided by LWTC, clinical prescribers are required to deliver Wrap Around Care to ensure prescribing is safe, effective, and appropriately monitored.
Patients are referred directly by their clinical prescriber using approved clinical systems.
Healthier You: NHS Long Term Conditions Prevention Programme
Yes, patients can self-register for the programme.
Please note that this NHS pilot is currently only available to patients registered with participating GP practices, and there are limited registration windows each year.
Only patients who meet the programme eligibility criteria will be accepted following self-registration.
To self-register, patients must provide the following information to LWTC:
- Personal details (name, date of birth, age, and contact details)
- Confirmation of a diagnosis of hypertension
- Confirmation that they do not have non-diabetic hyperglycaemia, type 2 diabetes, or a history of gestational diabetes (GDM)

- Identify any hearing or vision impairments
