Living Well, Taking Control & Liva Healthcare announce four NHS contracts to prevent Type 2 diabetes

Non-profit and diabetes support provider Living Well, Taking Control, and digital health tech company Liva Healthcare, announce four new contracts as part of the Healthier You: NHS Diabetes Prevention Programme (NHS DPP).


The NHS DPP is delivered by a group of providers including Living Well Taking Control (LWTC) and Liva. Liva is a subcontractor on the programme.


Patients from four regions and sustainable transformation partnerships (STPs) have access to the diabetes prevention programme, including; Derbyshire, Nottingham & Nottinghamshire, Northamptonshire, and Worcestershire and Herefordshire. This news comes after LWTC and Liva secured contracts with CCGs in Dorset, Shropshire, Wrekin, and Telford at the beginning of this year. Patients have already been welcomed onto the programme.


The programme


Over the next three years, as part of the Healthier You: NHS Diabetes Prevention Programme, LWTC will deliver group and one-to-one educational support session to patients over nine months. The programme is designed to enable long term behavioural change, giving patients all the information they need to make healthy choices. Sessions cover key topics such as understanding the risks of diabetes, behaviour change techniques, healthy lifestyles and mental wellbeing.


Alongside the structured education programme, Liva will deliver personalised digital health coaching to patients through their app.


Over three years, more than 34,000 patients who are at high risk of Type 2 diabetes in the regions will benefit from this partnership. The combined LWTC and Liva service will be provided free of charge to patients at risk of Type 2 diabetes through a referral from their GP.


Regular coaching sessions help patients tackle chronic conditions through sustainable lifestyle and behaviour changes. The programme has been proven to halt and reverse the progression of Type 2 diabetes.
